Online questionnaires and our form provider

The reflective questionnaires on this website are provided by Tally (Tally BV, Belgium), acting as a data processor on our behalf under a written data processing agreement. Your answers are transmitted over an encrypted connection and stored on servers in the European Union.

Questionnaire answers may reveal information about health or disability, which is special category data. Where you complete a questionnaire we rely on your explicit consent (UK GDPR Article 9(2)(a)), and you may withdraw that consent at any time by contacting us.

Answers are transferred into our secure records and deleted from the form provider once they have been reviewed. Email correspondence, by contrast, is not encrypted end to end, so please avoid sending sensitive details by email.

Information collected

Inclusive Ally Solutions collects only the information necessary to provide a professional diagnostic assessment. This may include:

Personal details (name, date of birth, address, email and telephone number).
Educational and employment history relevant to the assessment.
Previous assessment reports and supporting documentation.
Relevant health or neurodevelopmental information (Special Category Data).
Assessment notes, test record forms, raw scores, observations and the final diagnostic report.
Payment and invoicing information.
Why information is collected

Personal information is used to arrange and conduct your assessment; prepare your diagnostic report and recommendations; communicate before and after the assessment; maintain professional records in accordance with current SASC Assessment Guidance and the SASC Report Format; and meet legal, professional and financial obligations.

Personal information is never sold or used for third-party marketing.

Sharing information

Your information is treated as confidential. It will only be shared: with you; with another person or organisation where you have provided written consent (for example, a university, employer or Access to Work adviser); or where disclosure is required by law.

Special Category Data and consent

Information relating to your health or neurodevelopmental profile is classified as Special Category Data. Your explicit consent will be obtained before this information is collected and used for the purposes of completing your diagnostic assessment and producing your report.

You may withdraw your consent at any time by contacting Inclusive Ally Solutions. If consent is withdrawn before the assessment or report is completed, it may not be possible to continue providing the assessment service. Withdrawal does not affect processing already carried out or records that must be retained to meet legal or professional obligations.

Retention of records

After the retention period has expired, records are securely destroyed or permanently deleted unless there is a legal obligation to retain them for longer.

The electronic copy of your final diagnostic report will be retained securely for 6 years in accordance with current professional guidance and legal requirements.

All other assessment records, including assessment notes, questionnaires, consent forms, correspondence, test record forms, raw scores and any supporting evidence, will be securely destroyed once the assessment process has been completed, any agreed amendments have been made, and you have confirmed that you are satisfied with your final report.

Published assessment materials, test manuals and copyrighted test content are retained securely to maintain test security and are not disclosed or copied.

During a remote assessment, some tasks may require you to handwrite responses and send a scanned copy or photograph of your work. Once receipt of a clear digital copy has been confirmed, you will be asked to destroy the original document during the assessment. You should not retain any assessment materials or handwritten responses after the assessment has been completed.

Your rights

Under UK GDPR, you have the right to:

Access your personal information, including a copy of your assessment report.
Request correction of inaccurate information.
Request restriction or deletion of your information where applicable.
Withdraw consent where processing relies on consent.
Object to certain processing activities.
Complain about how your personal information has been handled.
Data protection complaints

If you have concerns about how your personal information has been handled, please contact Inclusive Ally Solutions using the details above. In accordance with UK GDPR requirements effective from 19 June 2026, a formal data protection complaints procedure is operated: complaints will be acknowledged within 30 days, investigated promptly and fairly, and a written outcome provided.

If you remain dissatisfied, you have the right to complain to the Information Commissioner’s Office (ICO).

Accessibility settings on this website

If you change text size, typeface, spacing, link highlighting, motion or read aloud in the accessibility panel, those choices are stored only in your own browser on the device you are using. They are never transmitted to Inclusive Ally Solutions, never used for analytics or marketing, and no one else can see them.

Because they stay on the device, they will still be in place the next time that browser opens the site. If you are reading on a shared, family or work computer and would rather leave no trace of your preferences, open the accessibility panel and choose Reset and clear my preferences. That removes the stored settings from the browser immediately.

Clearing your browser data will also remove them. Inclusive Ally Solutions sets no cookies of its own, and none are used for advertising or tracking.

The reflective questionnaires are displayed inside the page by our form provider, Tally. As a third party, Tally may set its own cookies or use similar storage on your device in order to run the form, remember your progress through it and protect against spam. These are set by Tally rather than by Inclusive Ally Solutions, and are not used to advertise to you. Tally’s own privacy and cookie information is available at tally.so. If you would prefer not to load the form, you are welcome to request the questions by email instead.
